Arizona Maine Coon Cat Rescue, Inc. ADOPTION AGREEMENT Please print and complete this form, then mail to address below.

ADOPTION FEE: Kitten up to 1 year: $150.00; Cats up to 10 years: 125.00; Older cats $100.00 Adoption fee may be paid by check, cash, or credit card. Checks should be made payable to AZMCCR.
Basic adoption fee rarely covers our expenses. All our cats are vet checked, and have received any treatments necessary. They have been spayed or neutered, are up to date on all shots, and have tested negative for Feline Leukemia and FIV.

Arizona Maine Coon Cat Rescue, Inc., 9123 N 115th Place, Scottsdale, AZ 85259.

In consideration of my receiving the MC/Mix cat: ____________________________

I HEREBY AGREE:

• to care for the above-mentioned cat humanely (including, but not limited to, water, food, shelter, vaccinations, and any necessary veterinary care).
• to relinquish custody only to AZ-MCCR. I will not sell, trade or dispose of this cat in any manner.
• to use this cat for no other purpose than as a family pet.
• to keep this cat indoors only, to not allow this cat outside except under close supervision, or on a leash. At no time will this cat be tied outside.
• to not declaw this cat
• to supply a scratching post for cats with intact claws.
• to not hold the AZ-MCCR Rescue/Adoption Coordinator or any AZ-MCCR members responsible for the conduct of the cat after adoption.
• to pay the adoption fee of $_____________

I FURTHER AGREE THAT AZ-MCCR:

• has not made any representations regarding this particular cat other than information given to them by former owners and the member's own observations of the cat while in their presence.
• does not warrant to know the parentage of cats in its care, and by this statement, I have been informed that it cannot and does not assure that the parentage of this cat includes any pure Maine Coon.
• accepts no responsibility for any liability or for any injury or damages to any person or property caused by this cat, as well as any cause of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damage.
• My signature acknowledges that I have read and understood the above, and will accept the above-described cat as part of my family.
